In Super Mario 64, the Eyerok boss battle starts with the boss resting as a pile of bricks on the floor, appearing to be part of the environment. After Mario steps on it, Eyerok reveals its true form, introduces itself with brief dialogue, and finally starts attacking.
If Mario stands on top of one of Eyerok's hands at the moment it would start attacking, a glitch occurs where he is immediately teleported to be under the hand as it begins stomping the floor. This gives Mario no chance to escape and automatically kills him within a few seconds.
